How do I open a business account with Milestones Federal Credit Union?

The following documents are needed at the time of account opening:

DBA / Sole Proprietor

  • Individual/s photo ID

  • IRS EIN Assignment Letter (if applicable)

Association/Organization (not for profit)

  • Meeting Minutes (Secretary notes, indicates position titles and names)

  • IRS EIN Assignment Letter

  • Photo ID for all account signers

LLC (Limited Liability Company)

  • Articles of Organization or Certificate of Formation

  • IRS EIN Assignment Letter

  • Beneficial Ownership Form

  • Photo ID for all account signers/owners/controllers

Partnership

  • Partnership Agreement

  • IRS EIN Assignment Letter

  • Beneficial Ownership Form

  • Photo ID for all account signers/owners/controllers

Corporation (including Non-profit Corporations)

  • Articles of Incorporation

  • IRS EIN Assignment Letter

  • Beneficial Ownership Form

  • Photo ID for all account signers/owners/controllers
